Nearly every expensive equipment failure in a dental office has a cheap maintenance task behind it that nobody owned. A vacuum pump that dies early because nobody cleaned the solids collector. A compressor tank that rusts through because the drain was never opened. A sterilizer that fails a spore test because the chamber was never descaled and the gasket was never replaced. This log turns those tasks into a routine with initials next to it, which is the only version of maintenance that survives a staff change.
How to use this template
- Print the daily and weekly sheets monthly and keep them on a clipboard in the sterilization area and the utility room, not in a binder in the office.
- Fill in your own equipment names and the manufacturer's intervals in the blank rows. The manual always outranks this template.
- Assign every recurring task to a role, not a person's name, so the routine survives turnover.
- Initials mean the task was done, not that it was assigned. Train the team on that difference.
- File completed sheets with your sterilization and compliance records; equipment maintenance records are part of an infection control program.

How to run this log
The log only works if it lives where the work happens. Put the daily and weekly sheets on a clipboard in the sterilization area and a second copy in the utility room. A binder in the manager's office guarantees the sheets get filled in from memory on Friday afternoon, which is worse than not having them, because it creates records you cannot rely on.
Start by customizing the equipment register and the intervals. Every line in this template is a common dental office task, but the authority for your practice is the manufacturer's instructions for use. Sterilizer descaling intervals differ between a gravity tabletop and a cassette unit. Compressor service intervals differ between oil-free and oil-lubricated. Waterline treatment schedules depend entirely on the product. Pull the manuals, write the real intervals into the blank rows, and cross out anything you do not have.
Then assign owners by role. "Sterilization lead" and "clinical lead" and "office manager" survive turnover; a name does not. Post the assignments where the team can see them. When someone leaves, you reassign a role rather than trying to remember what only they knew.
Who owns what
In a small practice, three roles cover the whole log. The sterilization lead owns instrument processing tasks: ultrasonic, sterilizer cleaning, indicator stock, and the weekly spore test. The clinical lead owns operatory equipment: waterlines, evacuation, handpieces, chairs, lights, and curing light testing. The office manager owns the calendar and the vendors: scheduling annual preventive maintenance, tracking registrations, filing service reports, and running the monthly records audit.
The owner also matters for the annual items. Annual preventive maintenance is the part that most often slips, because nothing visibly breaks when you skip it. Put every annual service on a calendar with a reminder six weeks ahead, not a mental note, and record the cost in the annual table so you can see what the equipment is actually costing you each year.
What good looks like
Good is boring. The daily column is initialed every day the office was open, including the days when things were hectic. The monthly sheet has findings written in the notes column, not just checkmarks, because a real inspection notices something: a gasket starting to flatten, a slightly slower chair, a dryer indicator that is not returning to blue. Those notes are how you catch a failure while it is still a repair instead of a replacement.
Good also means the service history sheet fills up over time and gets used. One more marker of a healthy log: an inspector or a prospective buyer can find it. Equipment maintenance records belong in the compliance binder alongside sterilization monitoring records. During a practice sale, a complete maintenance history is one of the few things that makes a buyer more comfortable paying for equipment rather than discounting it.
Common mistakes
Batch initialing. Someone fills in a week of boxes at once. It is fast, it is dishonest, and it destroys the value of the log. If a task was not done, leave the box blank and write why. A log with gaps and explanations is far more credible than a perfect one.
Using the template instead of the manual. Wiping a chair with the wrong disinfectant will crack the upholstery. Descaling a sterilizer with the wrong product can void a warranty. Running an oil-free compressor on an oil-lubricated schedule accomplishes nothing. Always confirm against the manufacturer's instructions.

Letting distilled water slide. Tap water in a steam sterilizer is one of the most common causes of chamber scaling and premature failure, and it is entirely preventable. The distillers and RO guide covers the options.
No plan for what happens when something fails. The escalation sheet at the end of this log exists for that. A team that knows the sterilizer comes out of service immediately after a positive spore test, without waiting for the owner to arrive, will handle a failure correctly.
